h4 stamping petition detials-urgent

regarding h4 stamping petition detials

I need some clarifications...
My wife is planning to attend for H4 re stamping as she went to India and
she is having her I797A which expires on 02/28/2010

We are trying to fill out the application through VFS website to take an appointment...
In VFS website there is a section to be filled in
"Petition Details", I am not sure what to fill out for the following questions:

1) Principal Applicants Name : (it is my name or my wife name or my company name)
2) Petition reciept number : (it is my name or my wife name )
3) Petitioner Name:* : (it is my name or my wife name or my company name)
4) Petition expiry date?... : (mine or her)

my wife is having I797A

IN my wife I797A paper
1) applicant column : her name
2) beneficiary column : HER name
3) IN I797A paper at bottom left conner near I 94 columne there is a petitioner detials in which
my WIFE name is mentioned.

Please advice.. Do i have to use my I797A detills or my Wife I797 A detials.

The approval that your wife has is from her change/extension of status application. (Which she signed and hence she is the applicant)

The petition that they are asking about is what your employer filed for you. (Which they signed and not you and hence the company is the petitioner)
