DS-156 Question after H1B Extension

Hello Everyone,

Recently, I got my H1B extension approval. Now, I am going to India to get my visa re stamped. Could any one please help me answer the below question in DS 156.

Note: I am presently staying in an Apartment with my wife in USA.

25. Name and Telephone Numbers of Person in U.S. Who You Will Be Staying With or Visiting for Tourism or Business

28. Who Will Pay For Your Trip?

Note: I have a previous B1 visa on which I have travlled twice to USA during the years 2005 and 2007. And I went to India to get married in Dec 2008 and came back on Jan 10 2009

29. Have You Ever Been in The U.S.?

30. Have You Ever Been Issued a U.S. Visa?
