H1B extension

Need help on H1B extension. Filed PERM in February 2009 but haven't received approval. H1B 6 years limit will expire in Feb.2010. After filed PERM, position and title both changed, same field though. Filed PERM with PH.D and state employee but right now is temporary worker as a MA degree. Company agreed to sponsor H1B extension. Do you think USCIS might reject because the big change?
