Client Letter for H1B stamping

Hi All,
I am coming to India for my 1st H1B stamping in dec end.
I wanted to know if anyone has a specific format for the client letter that i need to carry. Or else, if anyone can please let me know what all is needed on it, i would appreciate that.

Is there a list of all the documents (mandatory & recommended) that i should carry with me? I tried searching online but couldn't find it.

Is it ok if my client doesn't put an end date on the letter? Mine is an open ended contract which is extended every 6 months. Next extension is due in March 2010. Is the end date for the project necessary?
