H1 221g - Tatkaal passport application

I went to Chennai for my stamping and they said it was approved. But when I got me passport in the mail, it said my passport was damaged and I needed a new one.

I have a query regarding new passport application.

My permamnent address is in Bangalore and I have relevant proof for the same. But I have been in the US for the past 1 year.

1. Should I mention my US address as 'present address' or not?
2. Even if not, should I mention that I have been living there or is it not required?
3. They ask for a police station under whose jurisdiction it falls. What should I mention for a US address since it doesn't fall under the jurisdiction of any Indian police station?

3. If you give US address, you can put NA(Not applicable) for this.
