Is I-134 is required when L2 is entering aone

I am the L1B holder and right now I am in US. My family will travel in L2 visa in the month of Feb 2010. I have few concerns

Is I-134 is required in the port of entry of US ?

If I-134 is required, where I need to submit and how ?

Which other documents she should carry with her ?
