    what do we need to fill for the following in VFS while taking appoitnment?

    1) A resident of India/Bhutan OR An Indian Citizen Resident in US?
    >>> This should be "Yes".

    2) Are you applying for the same class of visa which is currently valid or has expired within the past 12 months?*
    (Are they talking about the actual Visa stamp on Passport or the extended I-797 which has the I-94 extension)?
    >>> This should be "No" since her visa expired more than 12 months ago.

    My wife was on H4 Visa in US till October 2009. She went to India in October 2009.

    1) H4 Visa stamp on her passport was valid unitl 30-SEP-2008. It expired in 30-SEP-2008.
    2) We've Extended her H4 Visa in 2008 till 02-FEB-2010 in US. I-94 was extended till 02-FEB-2010. No Stmaping was done.
    3) I got H1 extension until 02-FEB-2011 (Next Year)
    4) Applying for her H4 based on my new H1 extension

    what do we need to fill for the following in VFS while taking appoitnment?

    1) A resident of India/Bhutan OR An Indian Citizen Resident in US?
    2) Are you applying for the same class of visa which is currently valid or has expired within the past 12 months?*
    (Are they talking about the actual Visa stamp on Passport or the extended I-797 which has the I-94 extension)?
