Sample DS-156 and DS-157 Documents

Hi All,

I managed to find a filled form for DS-156 on this website. It also has a sample PDF duly filled form. This is really helpful.

I am not able to get a DS-157 filled sample. Is there one? Can anyone suggest the link if there is?
