H4 renew--when H4 never used before

I got Married in 2007 June.
Got H1 for both of us in 2007 Dec.

I travelled to US in 2008 March.
My Wife never traveled with me to USA.

I am still in USA. and my Visa (and her H4) expired in sep 2009.

Now, She is planning to come to USA.In the visa interview:
1) will there be any problem?
2) will the officer ask her why she did not use the previous H4?
3) Should I accompany her back home t g t vissa?
4) please help me with all info I should be prepared...

Few of my last questions.

1) myself changed the company and on New H1 which will expire in 2011 Sep.
I did not g for restamping.
so can my wife alone go f H4 stamping with all valid supporting documents.
should I accompany her back in India ??? are there any issues for a case like this....

2) Tax returns:
AS she is not there I have t pay Tax returns as Single.
W2 says I am married. But my Tax Consultant said as she is not there you cannot get ITIN so pay Single..(I have t pay lot of money to IRS) when she is back do a amendment.

This said: will this be a issue in the VIsa?
when I asked my Tax officer they said it should be ok...

But you being expects in Visaa..can you please let me know on this issue.
