  • H4-H1-H4 DS-160 form query

    Hi,
    I came to US on H4 visa and my H4 VISA expired in Sept 2008.

    I had applied for H1 work permit and got approved in Oct 2008 till sep 2011(but didn't get VISA stamped)."Got my SSN"

    I worked for 3 months and got COS to H4.Currently on H4.... Now I am going to India and I want to come back on H4.

    So I have some questions here to fill DS160 for H4 stamping.

    1. Should I give US social security number (which i had got while on H1) or ITIN or should i check does not apply option ?
    2. Have you ever been issued a US visa?
    a. Are you applying for same type of VISA?
    b. Date last VISA was issued?
    c. VISA number?

      I have checked the link to the guide but I could not get answers to the question posted here.

      Can someone kindly answer these questions ?

        1. SSN (If you've SSN, do you also have ITIN? how come?)
        2. So you don't know if you got a visa earlier?
        b. You should know
        c. Ditto
        d. Available on visa

          If you have an SSN you should enter it, it does not matter much anyway.

          Visa = what is in your passport, answer the questions for the last visa that was issued by any US consulate/embassy.
