H4 DS 160 help

All,

I am in US and I am filling my kid's DS 160 application for H4.

I have following questions and I couldnt find answers to them anywhere online.

1. At personal information, can Full name in native language be left blank? I remember when I went for H1, I left it blank and filled it on the printout before going for interview. What should I do now? Can I simpley check "Do not Apply"?
2. In travel information, "Person/Entity Paying for your trip" - for now I put as OTHER, and gave all my details. Is this correct? And if so, should I provide any affidavit of support?
3. In U.S Contact information, I gave my details but for "Relationship to you", should I put RELATIVE or OTHER?
4. Somewhere online, I learnt that I need to send LCA for interview. I dont think I have it now. Is it required?

Please help.
