Need help in filling DS 160 form - URGENT !!!!!

Hello,
I came to US on h4 in 2008 and recently i got a job offer from an university and they filed a non-profit h1b petition via consular processing and i received my approval notice(I797).So, before I start my work, I need to go to my home country(INDIA) and get my visa stamped.Now,I need some help in filling the questions in DS 160 form.Please help me!!

1. U.S Contact Information

- My question is, Do i need to fill these questions with my husband details or my employer details???

2.Work/Education/Training Information
Primary Occupation:
Present Employer or school name:
Address:
Month Salary in Local Currency:
Briefly describe your duties:

- My questions are,Since I didnot start my work here,should i give in the details of my university(employer) or shoud i fill NA(Not applicable) as i am staying here on h4 right now? If i have to fill in my employer details, what is local currency? is it rupee or dollar??

3. Enter monthly salary(in USD) : - Since I donot have any paystubs, I am not sure of my salary.However, I knew my annual salary. Can i divide it by 12 and fill it in the form..

Sorry for too many questions..Please reply asap as i m filling the form..
