I got my H1 in Oct 2008, but couldn’t work because of medical reasons.

Recently I have started applying for jobs, but without notice my employer cancelled my Visa in Apr 2010. Meanwhile I have applied for H4, waiting for approval.

I am planning to work in future , but I am very confused regarding COS from H4 to H1. Since my H1 is cancelled, do I need to apply for new H1 under new cap or just apply COS from H4 to H1 through a different employer as soon as I get a job. I have no paystubs.

Just h4 to h1 COS as you already cap counted . No paystubs required for h4 to h1 transfer.

yes. still you can do it. It will cost the same as filing new h1b. around $1500. And if premium then add $1000 more.
