HDFC Receipt Mistake (Wrong Form) - Need Help

My wife is in India and trying to get H4 Visa, for that she paid HDFC Visa fees on WHITE Form (Seems some bank assistant advised her wrongly)

Now we learnt that we need GREEN form for H4 Visa.

Please advise how to proceed!

Go to the bank and get correct one. It was bank's mistake, so they have to correct it.
