Need info for filling DS160 for H4

I and my wife are travelling to india and we have to get my visa stamped.
i have some of basic questions in DS160 for H4.
1)we got our 797 extended in US so my wife also have a receipt #.Do we need that on her DS160.
2)What should i mention in her DS160 in US Contact.
Should i enter my details or my Employer details.
3)My wife is a housewife.So Should the Occupation be Homemaker.
4) There is no option to mention H4 visa type so i assume that depandent of H1 is H4.is that correct on DS160 form for H4.
