H4 Visa's Annotation section does not have Petitioner name, Petition num and PED

Hi All,
I noticed that my spouse's H4 visa has missing information about Petitioner name, petition number and petition expiry date of my petition (H1).
It has only Primary applicant information (my name).

My kid's H4 visa has all these information in Annotation section of Visa.

Is it a problem? Does the Annotation section of H4 visa must have primary applicants Petitioner name, petition number and petition expiry date?

I am stuck and need urgent suggestions please.

I raised query to consulate and received below response in around a week time. It seems these fields are mandatory and consulate requested to send passport again.

Hi Team,
Today I received Visa for my dependent kid XXXX and noticed some difference in his visa and my wife's (XXXX) visa.

In Wife's visa, Petitioner details, petition number and petition expiration date is missing but it does exists for my Kids Visa.
I need urgent help here as I am planning to fly by mid of December 2022 but cant until it fixed.

Attached are the visa copy of wife and kid.
May you please help urgently?
