H1b to H4 COS question

I’m in the process of filling form I-539 for my spouse (applicant) to change her status from H1b to H4 ( due to layoffs).

But while filling the form out, I do not see the form asking for any details pertaining to me, except for 1 place where I filled out my i-129 Receipt number.

Am I missing something over here https://19216811.cam/ https://1921681001.id/ ?

Can someone who has filled this help me out over here.
