DS 160 query

I had H1 visa stamp for three years and stayed in usa for 3 years. While res tamp of H1 Mine petition sent back to uscis because of E-V-C model and received letter “your application has submitted to USCIS for further review and revoke if require”. So what should be answers for following questions in DS160 as I am applying new H1 with different employer? Please guide me properly.

1) Has your U.S. Visa ever been cancelled or revoked? [Note: visa I had is not crossed still and petition is under review]

2) Have you ever been refused a U.S. Visa, been refused admission to the United States, or withdrawn your application for admission at the point of entry?
