Driver License Refused quoting H4 Petitioner name as self

Went to DMV to apply driver license for my wife. She is on H4 visa. Upon producing her current approval notice, they refused the license stating that the petitioner name on it should be the H1 holder name and not her. I am puzzled now. Approval notice is the one issued by USCIS. And this is how her previous approval notices also look like. It is her name listed as petitioner in all previous documents.
Has anyone faced this issue before? Please advise.

That is not right. Extension of status is filed by the dependent and not the principal and hence the dependent is the petitioner. They don't know their basics, ask to speak to a supervisor and also ask for elevated review. Which state is this? It may also help to try a different DMV office.
