DS 156 and DS 157

Can any one (who changed from F1 to H1B) post DS 156 and DS 157 samples here. I saw the DS 156 in this website (immihelp), but its for people coming from India. I would like to see a sample of both the forms submitted by a person who is in the US and went from F1 to H1B. This is for my H1B visa stamping purpose.
