How is child age calculated for H4 ?

My nephew was born on Jan 2, 1992.
According to USCIS, the child has to be under 21. But, there are some interpretation that anyone entering 21 is not considered as child.

Come Jan 3 , 2012 , will my nephew be out of status ?
Or does he have 1 more year till Jan 2, 2013 ?

The common terminology used in India as being in the 21st year is not the global standard. Age 21 means 21 years completed.
