I-94

I am working in US on H1B.
when i visited india in June 2010, on return, I was issued an I94 [PortofEntry I94].
Since then i applied for H1B extension which was approved and i received I797A with another I-94 [I94 on 797A]

Now I am visiting india again this month. Do I have to return both the I94s [ The PortofEntry I94 and the newer I94 on 797A] ?

Also, I never returned the previous I94 on the I797A of 2004-2007. However i did enter and leave US 3 times after that. I even had the VISA stamping for that I797A. Do i need to do anything to correct this mistake ?

>>> You need to return all the I-94s. Otherwise proper exit entry will not be registered against the I-94 that is not returned and it can lead to issues.
