Query for COS I539 cancellation/ withdrawal

Hi,

Does any one have a template for the cancellation / withdrawal request for I-539 change of status request. Whom should the letter be addressed to and should it be typed and Signed or written in the applicants handwriting.

There is no real format. Address it to USCIS. Typing or handwritten, either is fine. It should mention the application receipt number, brief reason for withdrawal and request for further action if any. Attach copy of receipt notice. It should be signed and dated by the applicant of course. Keep a copy for your records.
