appointment h1b and h4 stamping together

My H1 Petition have been approved. I seek web appointment h1b.

My Questions Are :

(1) Together interview with my family and for fees

Do the new delhi embassy allow for together appointment i.e. interview for h1b and h4 to be stamped together ?

If i make appointment together with my family. Is it the fees would be charged (Rs.4400/ and Rs. 2200/- per candidate) and for childeren appx. Rs.3100/- per childeren.The fees would gone if the h1b refused even for the dependents in the case of together appointment. Kindly Confirm.

(2) Regarding Interview

Do the US embassy New Delhi, would take interview first of mine, to get approved h1b and then for dependents to approve for h4, in case of together appointment.

(3) Can It be Possible
=================
If, together with my dependents, I got the h4 and h1b visa stamped. Can I leave ahead of my dependents to usa and after settling there for 6 months, I want to call my family, which is already stamped for h4.
