Help on Visa H4 (Dependent Visa)

I am married and have passport with old name (maiden name) with valid H1B stamped. I want to apply H4 Visa (Dependent Visa) for my kid as well as for my husband.

My kid does not have passport so I am going to apply for the same. Since her birth certificate mentioned my new name (Same first name, middle name: husband’s name, New last name). So her passport will probably have my new name. Would it create problem for getting H4 Visa (Dependent Visa) for her?

My husband’s passport is recently expired. He is re-issuing the same.
Do my husband need to include my new name OR old name (maiden name)?

Please suggest.

1. That is not a problem. Just have your marriage certificate handy to show the name change.
2. He should include your new name.

Even though it is not required, you should consider changing the name in your passport to your married name to avoid any confusion in future.
