Hi!

question 11 of this form reads:
Have you ever before applied for employment authorization from USCIS?

I currently hold a H1B working visa and I did apply to the consultae to get that visa, but not directly to the USCIS. So the question is, should I answer yes or no?

Your answer should be ‘NO’.
H1B visa is a non-immigrant visa and Employment Authorization is for immigrant visa (e.g., Green Card).
