What documents do you need to apply for SSN ?

For a green card holder, to apply for SSN, is it correct that he will bring these documents ?
1. His non US passport
2. His green card
3. The SS application form (SS-5)

Is there any other document that you need to bring ?
Do we need to bring our birth certificate ?

Reading the application form, it says for proof of your identity, you must provide a:
●U.S. driver's license; or
●U.S. State-issued non-driver identity card; or
●U.S. passport
which we do not have yet.

Also, on the SS website, it says this:
To prove your age, present your birth certificate if you have it or can easily obtain it. If not, we can consider other documents, such as your passport or a document issued by DHS. --> Can this be the foreign passport ?

Foreign passport and GC is sufficient.
Also add proof of address such as bank statement or utility bill or lease or house title etc.
The SSA can give you a list of required documents when you go to them. They may also tell it on the phone.
