L2 EAD Documents Required

Hi - I'm planning to apply for L2-EAD card, please advice whats the procedure & documents required.
To my knowledge so far (considering I'll do paper filing) below is the checklist, please correct if I'm wrong:

1. I need to fill up & submit I-765 Form & I-765 Worksheet
2. L1A Petition Approval (I767) photocopy. [Do I need to send I129 of L1A as well?]
3. L1 Passport & Visa - photocopy
4. L2 Passport & visa - photocopy
5. I94- for L1 & L2 both
6. 2 photographs for L2 holder
7. Is SSN card of L1 or L2 require???
8. Marriage Certificate

Again, what is mode of payment for filing Fee of $380?
Also, considering the Expected EAD application for H4 visa holders, is there any new SLA published for processing timeline?

Check if you really need I765 work sheet.
L1 petition approval should be sufficient.
Copy of SSN not required, filling the inforamtion is sufficient.
You can give cashiers check.

Is there any reason for paper filing when you can do online filing and pay the fee directly. Once you do online filing, you need to send the supporting documents.
