Required documents for visa L2 and additional question

Could you please provide with list of all required documents for visa L2. I found some list in internet, but I want to make sure...

1.question about Federal tax returns (IRS Form 1040 ): Can I provide tax return transcript or I need to print form 1040?
2. if I had previous visa - BLANKET L2, but now I am applying for individual L2. It will be as same class?
