Employers from certain states are required to report wages and pay taxes due on a quarterly basis. All gross wages for each individual who worked for the employer for that quarter must be reported.

      Wages include:

      1) Hourly wages, salaries and commissions
      2) Meals, lodging and other payments in kind
      3) Tips and gratuities
      4) Remuneration for services of an employee with equipment
      5) Vacation and sick pay
      6) Separation and dismissal pay
      7) Bonuses and gifts
      8) Payments in stock
      9) Contributions to deferred compensation plans, including 401(k) plans

Check with your employer and they should have it. This something the employer and the attorney should be able to handle and should not be your concern as this document is company confidential document.
