1-693 form question

Which name (married last name or maiden last name) can be used to submit the I-693, Report of Medical Examination and Vaccination Record?
My vaccination records are under maiden name. I am currently working on name change process as well. Any guidance is great.

Use the name on your passport, and/or on your visa. But if you got married after arriving in the U.S., the married name is okay, but you have to show the medical office your passport for them to complete the firm.

What name change process are you talking about? Once married, a woman has a legal right to use the married name without any extra processing (in U.S.)
