My mother has a multiple entry USA visitor visa. She is planning to visit us for 4 months. If during this visit, she were to visit Canada for 4-5 days, will her re-entry to USA give her a new I-94 with an entry of another 6 months or will the re-entry be only for the remaining period of her visit?

Earlier that used to be the case. But due to misuse the Immigration officer are nowadays very strict on giving a new I-94 upon re-entry. In most likelihood when she leaves Canada for a very short duration such as 4-5 days her existing I-94 will not be taken and she would have to use the same while re-entering as they would not issue a new one.
