Visitor Visa - 2 questions

Hi,

1)I would like to know what copies should be Notarised or only I-134 needs to be notarised ? I have taken copies of Passport , Pay-slips, Bank Statements,etc but should those xerox copies should be notarised or is it fine to attach copies like that only ?

2) Do we have to attached any Originals ?
