Info required to fillout online appt application

Like most of you I am also getting frustrated and losing patience checking tts for visitor visa app for my parents. (Chennai - telugu).

If and when I get lucky and find an opening, I want to know what information does the online appt form need. All I have at this time is passport number, dob, place of birth, address, in other words copy of passport first page.

It would be really a shame if I lose the appt date just because I do not have the info required filling online application.

Can anyone please post the information I should have on hand for filling out the appt application online?
