Do we need to fill DS-157?

Hi,

My mother has an appointment on November 10th at Chennai consulate. We took at the appointment through ttsvisas.com in june.

According to the new vfs site everyone has to fill DS-157.

Though we took the appointment from ttsvisas.com, do we need to fill DS-157 for my mom?
