    Hi,

    My sisters last name on her passport is her maiden last name, on her husbands passport (under the field of spouse name) it is with her husbands last name and on her son's passport its her maiden last name. Don't know how that happened but thats the way it is as of now.

    So I have a couple of questions regarding this issue -

    1. First of all is this going to be a problem. And if yes how do we fix it ? She already has a notarized letter saying that her last name is changed to her husbands last name. Would that suffice ?

    2. Should I put her name as it is in husbands passport on her D157 form in questions (Q #8 & 9) about aliases and other names ? Is it necessary ? Would that help ?

    That's not a problem.

    Visa will be issued as per the name in the passport.

    Prepare your documents as per her name in the passport.

      So should I put NA in Q 8 & 9 ?

      I thought I would put all the variations of my sister's names from her husband's and son's passports in fields for Questions 8 & 9 in her DS156.

      Q8 is "Other Surnames Used (Maiden, Religious, Professional, Alises)"

      Q9 is "Other First and Middle Names used"

      Or should I just put NA in both ??? I thought it made sense to put the other names there for consistency but if it serves no purpose then I'd rather put "NA" there.

      Please advice.
