k3 visa from within us

i have a german citizen as a wife, i am a us citizen..if we go to the us , her on here 90 passport and we decide we want to live in the us can i fill we file for the k3 visa for her while she is in the united states and if we can , will she have to leave at the end of 90 days or id there a way she can stay during the process untill she gets a temp green card...please help ...and does anyone know how long a cr1 or k3 takes to process in germany
