DS 160 : "Other Name" Question For Mother

Hi Guys ,
My mother had following name

Name Before Marriage : Premlata ABC ( School leaving Certificate is with this name )
Name After First Marriage : Prema UVW ( She doesn’t have any document on this name . Expect my passport have her this name as I am Son from First Marriage ).
Name After Second Marriage : Premlata XYZ ( her Passport is with this name )

I have question while doing DS160 application she should mention Name Before Marriage (Premlata ABC) and/or Name After First Marriage (Premlata UVW ) in “ Other Name” Section of DS160 or not required .

If required which name she should mention in Other Name” Section of DS160 and does any affidavit is needed and please help with format .

I would list her current name as the name after 2nd marriage which is on her passport and list her previous married name as it shows on your passport as other name.

Do I need to also list her Maiden name ( Before Marriage ) also in other name
