I am on a workpermit in canada from india. I need to go to US to visit my cousin who is also on a workpermit in wisconsin. I do not know how to proceed further, please help.

I am in ontario belleville, which is the nearest US embassy and where will i get the application form, do i have to go in person and what are the chances of my visa getting approved?
