Can I fill/sign ds160 from US on behalf of parents in India? If yes, do I need to mention that the form is prepared by someone else i.e. myself. Do they track the IP address from where the form was filled and signed?

Yes, you can do so & of course mention it (while you submit) that you filled in their forms - its always better to be truthful & children can definitely fill-in forms for parents.

It really doesn't matters who filled-in the form - as long as the information provided are accurate & correct.
