Help please in visit visa

Hello All,

I am planning to visit my father & mother in law who are living in US. I want to visit with my wife and currently we live in UK.

i know I have to use B2 form but what I need to know that because we want to go together and also apply together so would we be using same form as I cant seem to find any such section other than mentioning little details for my spouse

Would I need to fill a separate form for my wife even but then book a same appointment. I am little confuse

Please can someone advise me and i will appreciate
