DS 156 - Help Required

Hi,

My uncle (Father's brother) has an appointment in Chennai consultate for Sep 25th. I am his sponsor and will be providing all the required dcouments. Can anyone help me what I should fill in DS 156 for Quesiton 37. Any quick response will be greatly appreciated as I will be sending the documents to India on Wednesday (09/13)
