I-134,Affidavit of Support

My in-laws visa interview is sceduled on 18th Dec in Mumbai.

I am filling I-134,Affidavit of Support form.

Do i need to send two seperate I-134 form fo my father-in-law and mother-in-law or just one I-134 form for both??

Please advise.
