I have a question on who should fill DS-157 form. On the website it says anyone between 15 and 55 should fill the form. My mom is between 55 and 56...does she need a DS-157 form?

while filling the application form, if required VFS will make you to fill the DS157 form. After filling all the rqd forms you wld be able to get an appointment

Thanks for the information. After I fill the application does it give different dates than what it shows on the check availability link. I am trying to get an appointment at Mumbai consulate but there are not dates available when go to check availability. Can the information on DS-156 be changed after it is filled?
