I-134 Question - Age of dependent?

Hi,
I have two questions

a) We have a baby who is about a few months old. I need to list her as a dependent in I-134 , should I put her age as "x months" in the dependents column?

b) I sponsored my in-laws last year and should I put their names under the column where it asks for previously submitted affidavits?
