Hello, Does anyone know where I should send the I-134 form and supporting documents to? I have a green card and am living in the US now, should I send the form and docs to USICS? If so, what is the address, I live near Atlanta, GA. Or should I send the I-134 form and supporting docs back to home so that they can send with their docs (such as DS-156 ...) to the US consulate? In short, I really don't know where to send the I-134 form and supporting docs to. Any help is very much appreciated.

I have the same question, but it's 10 years after this post was originally made. The I-134 instructions indicate I should check the USCIS website for the mailing address, but the website says I should check the instructions. There's also a note that the address might depend where the person I'm sponsoring is living. There's also another note indicating that once the form is approved, I am to send the completed form to the person I'm sponsoring to have available during the interview. There seems to be a few key details of where, when, and how to send my completed form I-134. Am I reading too deep into this, do I just need to send it to the person I'm sponsoring, or do I also need to send it to the US consulate in Shenyang(close to my mother-in-law step-mother who I am sponsoring), or just send it to the USCIS lockbox here in USA?Last edited by Andy500; 02-19-2018, 12:55 PM.

I-134 form is used for different purposes. That is the source of your confusion.
But for visitors visa, send it to the person whom you are supporting. He/she will take it to the consulate at the time of appointment.Immihelp Support
