DS157 and DS156 querry

1)I have filled DS157 form and for question 18 (Have you made specific travel arrangements) i have entered NO as we have not made any arrangements. Does it make any difference and is it always better to block the dates and get a itinerary and hence fill in question 18 as 'YES'. or is it okay to say 'NO'.If its better to mention is, i beleive we can just block the tentative dates, including return dates and get the itinerary, is that true. Please suggest as to how important is this.

2)I understand we need to affix photograph on DS156 form, can someone please suggest if we need to paste the photograph or just staple or attach it to the form

2) I've seen people show up with the form without the photo affixed - the guy checking your papers will either staple or glue it to the form.
