surname issue

Hi,
I was filling the DS156 for my Mother.
She doesn't have a surname in the passport. So what shall I write in the surname feild of DS156? Shall I write "FNU" or NA? Please help me out.

I am sponsoring my mother, sister and her kid all by myself. Do I need to send a three different sets of my documents to all of them? For example, do I need to send my bank statements,PP copies,employment letter and other docs to three of them individually?

Please help me out with these questions

If they are all going in the same appointment slot, send one set of documents, except I-134 which should be one per person.
