last name query in birht affidavit

Hi,

I have a query on Birth affidavit.

I have last name as married name in my passport. I do not have a original birth certificate. I need to get a birth affidavit. What last name should i use in my Birth Affidavit . the married name as in my passport or my maiden name.?

It is better to mention just your first name and the parents' name column should have the complete name of your parents.

use the full name before marriage and also add aka (also known as) the married name
