I-94 form filling ....

On the I-94 arrival record form, section 9 which says "City Where You Boarded" - if travelling from Bombay-Amsterdam-Minneapolis , what city should one enter for section 9?
and also what should be the "flight name and number" for section 7..?
should the flight number be of BOM-AMS or AMS-MSP?

What should be done if the space provided is not enough to fill the Given name ? can we add boxes by pen?

Do not ask the same question more than once:

1) Bombay
2) AMS-MSP
3) Just add letters, not boxes with letters.
